My first answer is that for fiction reading I will choose a text which is slightly below the students level, so there are not too many unknown words. If there are lots of unknown words I won't choose that text.
Second, vocabulary teaching I will do in regular classes. Fiction classes (as part of an extensive reading program) I will focus more on character, plot and "feeling".
Third. If I do feel that vocabulary teaching becomes important even in a fiction reading program I will devise a one-off special vocabulary class, based on tips on how to infer meaning from context and, very important, how to not worry too much about unknown words, how to grasp the main points about plot and character even when you don't understand every word.
